If you have Doom2, run "Cthulu.bat".  If you have Legacy run "LCthulu.bat"
and if you have ZDoom run "ZCthulu.bat".  Essentially the command line is
"Doom2 -file Cthulu.wad -deh Cthulu.deh" so just ensure to unzip everything
into your Doom2 directory.

And, yes, I am aware that 'Cthulu' is actually spelt 'Cthulhu' but does that
really matter?!
